FEES: The fee for Cardiac CT Training Course (Thursday through Sunday, April 2 - 5, 2009) is $950 for physicians if your registration is postmarked by March 2, 2009. After that date, the fee is $995. For Residents and Fellows the fee is $850 or $895 if your registration is postmarked after March 2, 2009. Retired, Military, and Technologists the fee is $750 or $795 if your registration is postmarked after March 2, 2009. A letter of verification from the Department Chairman must accompany the application of anyone wishing to qualify for the reduced rate. Enrollment will be confirmed on the basis of registration fee. We do not process registration without payment. We cannot guarantee course materials on site for a registration received after March 2, 2009.

REFUNDS: Cancellations received in writing by March 2, 2009, will receive a refund minus a $75 handling fee. There will be no refunds for cancellations received after that time.

Registration : Registration will take place on Thursday at 7:00 a.m. in the foyer of the hotel.
